How to use it

1. Mark the hole

After holing out—or while moving to the next tee—record the essential outcomes from the hole.

2. Keep it factual

Do not write swing thoughts or explanations. Circle the result, enter the few necessary numbers and move on.

3. Review the pattern

After the round, total the three-hole summaries and look for repeated tendencies rather than isolated mistakes.

4. Choose one priority

Turn the strongest repeated pattern into the next practice objective.

Tee shot

Record the club, whether the shot was playable, the miss direction and any penalty.

Approach

Record yardage, club, result and miss direction:

  • GIR: Green in regulation

  • Q: Quality miss

  • N: Non-quality miss

  • T: Trouble

Around the green

Rate the recovery lie as easy, medium or hard and record whether you got up and down.

Putting

Record first-putt distance, total putts and any three-putt.

Commitment

Circle a simple mental-performance score:

  • 1: Fully committed

  • 2: Partial or unclear

  • 3: Steered, rushed or forced

Traditional statistics do not always tell the full story.

A missed fairway may still leave a clear approach. A missed green may be the result of a smart target and a perfectly acceptable shot. A green in regulation can still produce a costly three-putt.
